MEG — Minced Expansion Graft
- Problem
- Superficial soft-tissue trauma, common in older patients, where the avulsed skin is treated as waste and debrided away.
- Existing answer
- Conventional management discards the injured tissue and waits for the wound to epithelialise from its margins.
- New approach
- Salvage the debrided tissue, mince it to expand its area, and return it to the wound as an autologous graft.
It states a general principle for the wound surgeon: no tissue should be discarded, but salvaged and placed back into the wound.

2016
The skin tear reconsidered
- Problem
- Skin tears in older patients, treated as random-pattern flaps whose damaged blood supply must be protected.
- Existing answer
- Preserving a flap whose perfusion is already lost delays healing and risks losing the tissue anyway.
- New approach
- Ignore the injured blood flow and treat the tissue as a skin graft instead, bolstered rather than merely re-approximated.

Traumatic skin graft and tissue salvage
- Problem
- Geriatric trauma with large avulsion injuries, where open wounds heal slowly and dominate quality of life.
- Existing answer
- Standard wound care leaves substantial defects to close on their own, over months.
- New approach
- A protocol combining traumatic skin grafts with minced expansion grafts — the earlier technique carried into a harder problem.
