1. New York · from 1997

   Reconstructive foundation

   Training at NYU Medical Center, Institute of Reconstructive Plastic Surgery, 1997–2003. The early work was reconstructive: microsurgery, hand and extremity problems, replantation, and the reconstructive clinic at NYU Downtown Hospital.

   The published record from that period reflects it. Nonextremity replantation: the management of amputations of the facial parts and testicle appeared in Clinics in Plastic Surgery in 2007 — the management of amputations outside the extremities.

   This chapter matters because it is the one most easily lost. The reconstructive work was not a later turn; it was there from the start.

2. New York · Fifth Avenue

   Aesthetic practice and media

   Early in practice, Dr. Klapper ran the reconstructive clinic at NYU Downtown Hospital while building a busy aesthetic practice on Fifth Avenue in New York City. That practice later brought television and broader media exposure.

   The New York chapter also included a busy Fifth Avenue aesthetic practice and substantial television, radio, newspaper, magazine and reality-television exposure. It is part of the record, and it is not the current work.

3. 2001

   Ground Zero response

   Participated in the deployment of the Bellevue Hospital plastic surgery team for Ground Zero triage response following the September 11 attacks.

4. South Florida · 2013 to present

   Hospital-based reconstruction

   He ultimately left the aesthetic and media world and returned the center of his career to hospital-based reconstruction — trauma, limb salvage, complex wounds, teaching, and surgical innovation.

   The centre of gravity is now trauma reconstruction, limb salvage, complex wounds and hand and upper-extremity reconstruction, at Delray Medical Center.

5. 2016 to 2026

   Tissue salvage and technique

   A recurring pattern of reconstructive problem-solving, each step published: minced expansion grafting, the skin tear treated as a graft, traumatic skin graft and tissue salvage, and most recently SMART muscle repair.

6. Beijing

   Teaching Microsurgery in Beijing

   Spent one month running the university's microsurgery laboratory and providing hands-on microsurgical training to surgical residents.

7. 2026

   Research and systems

   The most recent chapter turns the same habit on the conditions the work is done under: physician economics, reimbursement benchmarks, the No Surprises Act and independent dispute resolution.
